Youth ACT Director Access: Supports For Living Middletown, New York
This presentation outlines the 18mo. journey of how an all bilingual team of a prescriber, therapists, and care navigators set out to comprehensively address the needs of underserved community members diagnosed with an SMI and/or SED through Rapid Engagement And Community Health (REACH).
Presenters will share Access's Project REACH model for focusing on engaging underrepresented individuals that has resulted in the REACH team providing over 6,100 clinical services and 2,400 care navigation services across the urban, suburban, and rural areas of New York's Orange and UIster Counties, as well as lessons learned during implementation and post grant funded sustainability plans.
