The Acute Community Service Team (ACST) was created at The Village for Families and Children as part of SAMHSA funded CCBHC services. The team quickly realized that ACST clients required a more intensive needs and basic life skills assessment. The team used a software program that organizes numbers and data with formulas to convert the assessment from a Likert scale. Via the digital poster, we hope to demonstrate the tool, share individual and aggregate client baseline, progress, and outcome data, highlight improvements made during repeated implementation, and demonstrate the potential to save time and effort by using technology.
