Creating effective diversion programs from the CJS into community-based services requires CBHOs to develop collaborative relationships with Criminal Justice partners. This workshop will help organizations:
1. Build a Team Based, Trauma Informed Model for crisis intervention and system transitions to decrease incarceration, increase
access to care and create a data collection system that supports CQI.
2. Establish effective communication and collaboration between CBHOs and the Criminal Justice System.
3. Develop care coordination strategies to facilitate successful transitions from incarceration to community-based care.

Learning Objectives:

Identify and partner with stakeholders committed to preventing the incarceration of individuals with behavioral health conditions.
Align workflows and care pathways between the CJS and CBHOs.
Communicate effectively across disciplines and domains.
