Transition planning for individuals releasing from jail with co-occurring substance use and mental health disorders improves behavioral health outcomes and reduces recidivism, according to SAMHSA and the Sequential Intercept Model (SIM). The Bernalillo County Resource Re-entry Center (RRC), is an innovative, social model program that combines transition planning and re-entry services to individuals releasing from the county jail.

Learning Objectives:

  • Assess the value of a transition planning team to reduce recidivism, connect clients to community services, and reduce costs to the community regarding public safety.
  • Recognize that jails act as an opportunity to provide high risk individuals with care beginning at booking and serve as a connection for clients to receive services upon release.
  • Demonstrate the positive impact of transition planning and a re-entry center to a national audience to encourage implementation of the programs in local communities.
