Integrated health care homes address the disproportionate morbidity/mortality in populations with serious mental illness by improving access to and coordination of care, improved health literacy, autonomy and adherence to care plans and improved overall wellness. In 2022, Lutheran Family Services entered a partnership with United Behavioral Health/Optum to become an Integrated Behavioral Health Home provider for their Medicaid MCO. We are accountable for an individual’s total care to address physical, behavioral and social health of attributed high utilizing or difficult to engage individuals. This presentation will cover the first year of this pilot by an outpatient, state-wide behavioral health organization.
Learning Objectives:
Explore scope of services and populations served at LFS and identify common mental/physical health issues of the populations served by this organization and within the Integrated Behavioral Health Home program.
Presentation will review the planning and implementation process involved with the IBHH pilot at LFS and identify areas of success and failure throughout this process.
Presenters will discuss, based on lessons learned in this pilot, strategies to improve outreach, engagement, health literacy and self-management in an IBHH client population.
