TP35 - Positive Progress Toward Prevention

Presentation Description: *Integrating nursing services across programs to identify health related concerns and promote wellness in Behavioral Health
*Planning/implementation of new services such as Limited Laboratory Services and monitoring of vital signs for early identification of medical concerns as well as to help meet HEDIS measures/quality measures for established clients
*Increasing nursing services including Health Monitoring/screenings and inclusion of preventative measures such as distribution of harm reduction supplies and sexual health tools such as condoms, etc. in conjunction with HIV/HCV screenings through Limited Lab Services
*Coordination of care across programs/providers within the agency to ensure continuity of care/services for clients.

Learning Objectives:

  • Identify new services being implemented to improve health outcomes for new and existing Behavioral Health clients
  • Discuss the benefits and potential uses for nursing staff/services in improving the overall health of the people we serve.
  • Analyze data prior to implementation vs current data to identify current progress as well as goals for future improvement.
