MP21 - Leveraging Measurement-based Care to Enhance CCBHC Care and Outcomes

Data-driven decision-making to support individual treatment plans as well as to inform program level improvements is central to the Certified Community Behavioral Health Clinic (CCBHC) model. In practice, however, many organizations struggle to systematically implement standardized outcome measurement and data-driven practices. Measurement-Based Care (MBC), the practice of routinely measuring patient-reported data to inform decision-making, is a patient-centered practice that supports organizations in achieving a data-driven culture. This poster will describe a recent CCBHC implementation to demonstrate how MBC was leveraged to both meet CCBHC model requirements and improve overall quality and outcomes.
