This presentation summarizes a collection of evidence-based tools designed to be deployed within behavioral health services settings. Using longitudinal research and evaluation data from a community mental health agency, a series of clinical tools have been created to support the management of the physical co-morbidities and health disparities and affect people with behavioral health conditions. Participants will be able to access these materials immediately as well as connect with the program team for ongoing workforce trainings, technical assistance, and supports at no cost.

Review the impact of medical conditions, nature of health disparities, and treatment needs among people with behavioral health conditions.
Describe a coordinated series of evidence-based clinical health tools that can be used to promote engagement, health self-monitoring, and wellness tracking among clients.
Demonstrate the ways in which clinicians can implement meaningful processes and promote skills for tracking key gold-standard medical indicators within behavioral health settings.
