A crisis occurs — then what? Through its continuum of care, LifeWays in Jackson and Hillsdale, Michigan, offers a unique perspective and widespread array of services for a person-centered, customizable pathway to connect individuals to the most appropriate care and services from the point of crisis. Learn how this Certified Community Behavioral Health Clinic (CCBHC) Demonstration Site has worked to adapt its models within two distinctly different counties to meet the individual needs of each community.

  • Identify the provider network model and its impact on service arrays as both a CCBHC grantee and state-funded CCBHC Demonstration Site.
