This presentation will describe an innovative initiative that aims to improve access to community behavioral health services, through a partnership between Certified Community Behavioral Health Clinics and health systems. Targeted for individuals who are seeking care from an emergency department, this project is a regional collaboration aimed at strengthening the ecosystem of care across four health systems, two CCBHCs and community-based behavioral health providers. Audience members will understand how CCBHCs and health systems can collaborate to connect patients to care, and describe a replicable model to improve transitions of care for a population accessing the ED with behavioral health needs.

Learning Objectives:

Discuss how a collaborative clinical redesign project focused on improving transitions of care across partners is conceptualized, designed and implemented.
Identify key takeaways and initial lessons learned around supporting patients who visit the emergency department in connecting to community-based behavioral health services.
Demonstrate how Certified Community Behavioral Health Clinics (CCBHCs) and health systems can work together to connect patients to community-based care.
