As the main primary care provider for underserved populations and communities, health centers use care teams to deliver care and services in a centralized health home. This health center panel will spotlight various primary care and behavioral health integrated services targeting a range of medically underserved patient groups and the best practices learned along the way. The panel will provide an overview of primary and behavioral health care integration while discussing strategies for care coordination to support patients and the opportunities for partnerships to help overcome workforce challenges.
