Katherine Schroeder, LMHC, MCAP, QCS, Process Change Consultant, David R. Swann, MA, LCAS, CCS, LPC, NCC, Senior Healthcare Integration Consultant and Valerie Westhead, MD, Medical Consultant of MTM Services will provide this workshop. Attendees will develop capabilities to support and increase the use of collaborative and episodic care competency skills, evidence-based practices for psychiatric practitioners, clinical, clinical supervisors and leaders. This will help teams to improve treatment outcomes and deliver better care. Strategies will include the implementation of evidence-based care pathways and align workflows.

  • Learn the difference between collaborative care and the traditional siloed model.
  • Develop short term objectives to motivate clients and instill hope.
