Housing needs are one of the largest challenges facing people served in Certified Community Behavioral Health Clinics (CCBHCs), and local housing providers are often unaware of the support opportunities available to their residents within the CCBHCs in their communities. Attendees will learn about the four layers of partnership possibilities between CCBHCs and housing providers: homeless outreach, referrals through care coordination, CCBHCs providing outpatient services to residents, and – in some states) – building Medicaid housing support services into state implementations of CCBHCs.

Learning Objectives:

Identify solutions available through the CCBHC model to meet the housing needs of individuals.
Examine the necessary partnerships that enable individuals to secure needed housing.
Analyze opportunities to build housing support services into Medicaid where possible.
