Presentation Description: More than 90 randomized controlled trials and several meta-analyses have shown the Collaborative Care Model (CoCM) to be effective at treating depression, anxiety, and other behavioral health conditions in primary care settings. After reviewing the essential roles and elements of CoCM, this session will review commons pitfalls in CoCM implementation and how to avoid them. Examples include failure to obtain buy-in from the right people at the right time and not understanding the necessary conditions for financial sustainability. Panelists will share opportunities, challenges, and lessons learned from assisting hundreds of organizations with Collaborative Care training and implementation.
